Other Abstract | Objective: To investigate which index may be used in the differential diagnosis between FH and FCHL. and the correlation of apolipoprotein and coronary heart disease (CHD) in FH family members
Methods: The FH、FCHL and normal control pedigrees which meet the diagnosis criteria respectively were recruited. We analyzed the lipid levels and renal function indexes between groups. The lipid levels were compared between groups of FH pedigrees, and the correlation and the predictive value of lipid profiles to the onset of CHD were investigated between the indicators and the onset of CHD.
Results:
1.Compared with the control group, the TC, TG, HDL - C, LDL - C, Apo B and Lp(a) were increased and Apo AⅠ/Apo B was decreased in the affected group of FH pedigrees and the affected group of FCHL pedigrees. Compared with the non-affected group of FH pedigrees, the TC, TG, HDL - C, LDL - C, Apo B and Lp(a) were increased and Apo AⅠ/Apo B was decreased in the affected group of FH pedigrees. Compared with the non-affected group of FCHL pedigrees, the TC, TG, HDL - C, LDL – C and Apo B were increased and Apo AⅠ/Apo B was decreased in the affected group of FCHL pedigrees. Compared with the affected group of FCHL pedigrees, the TC, LDL – C and Apo B were increased and Apo AⅠ/Apo B and TG were decreased in the affected group of FH pedigrees.
2. The level of BUN and Cr in the affected group of FH pedigrees were higher than that of control group and the non-affected group of FH pedigrees, the level of BUN in the affected group of FCHL pedigrees was higher than that of control group.
3. Compared with the normal group, the TC, LDL - C, Apo B and Apo B / Apo AⅠwere higher in the high cholesterol group; the TC, TG, LDL - C, Apo B , Apo B / Apo AⅠand Lp(a)were higher in the CHD group. Compared with the high cholesterol group, the TG, LDL - C, Apo B , Apo B / Apo AⅠand Lp(a)were higher and the Apo AⅠwas lower in CHD group.
4. The LDL-C, Apo B, Apo B/Apo AⅠand Lp(a) were positively correlated with the onset of CHD in FH pedigrees.
5. The biggest area under the ROC curve was Apo B/Apo AⅠ, and the optimal cut-off point were 0.75.
Conclusion: The levels of Apo B and the ratio of Apo AⅠ/Apo B were expected to become the index of differential diagnosis between FH and FCHL. The LDL-C, Apo B, Apo B/Apo AⅠand Lp(a) were positively correlated with the onset of CHD in FH pedigrees.
