Abstract目的 对直肠癌患者的DCE-MRI定量参数与Ki67、HER-2的相关性进行比较和探讨,分析其内在联系,提高DCE-MRI在临床中的应用价值。 材料与方法 对94例经手术病理诊断为原发性直肠癌患者,均于术前行动态增强MRI检查,获取肿瘤部位ROI的定量参数值。采用免疫组化检测分子生物学标记物Ki67、HER-2的表达情况,并对DCE-MRI定量参数与直肠癌组织分化程度、及Ki67、HER-2表达情况进行相关性分析。 结果 1、直肠癌患者不同组织分化类型的DCE-MRI定量参数Ktrans有显著差异,Ve、Kep无意义。Ktrans、Ve、Kep分别与直肠癌病理组织分化程度均呈正相关。 2、Ki67不同表达水平的DCE-MRI定量参数Ktrans具有显著差异,Ve、Kep无意义;Ktrans、Kep与Ki67呈正相关;HER-2不同表达水平的Ktrans、Kep有显著差异,Ve无意义;Ktrans、Kep与HER-2阳性表达亦呈正相关,Ve无意义。 3、淋巴结是否转移与动态增强MRI定量参数Ktrans、Kep具有明显差异;且与Ki67、HER-2亦呈正相关。 4、Ki67与HER-2之间呈正相关。 结论 DCE-MRI定量参数值能够反映直肠癌肿瘤微血管灌注量、血管的生成状态、病理分期、分级和肿瘤的恶性程度,间接反映直肠癌肿瘤组织的生物学行为;同时,分子生物学标记物Ki67、HER-2对于直肠癌的发生发展具有一定的促进作用。
Other AbstractObjective To evaluate the association between DCE-MRI quantitative parameters and Ki67 and HER-2 for rectal cancer. To improve the value of DCE-MRI in clinical application. Materials and Methods Gathering 94 cases of rectal cancer patients who underwent DCE-MRI. We measured Ktrans、Ve and Kep and detected Ki67 and HER-2 expression by SP staining.Then we analyzed the correlation between DCE-MRI quantitative parameters and expression of Ki67 and HER-2 in rectal cancer. Results 1、For patients with rectal cancer by histopathological staging, Ktrans were significant difference,others were not. Ktrans, Ve and Kep were positive correlation with rectal cancer pathological tissue differentiation degree. 2、Ktrans was significant statistical significance in Ki67,Ve and Kep were not, Ktrans and Kep were positive correlate to Ki67; Ktrans and Kep were significance in different HER-2 expression level,Ve was not; Ktrans,and Kep were positively correlated to HER-2 positive expression , Ve was not. 3、Ktrans and Kep were significant significance between lymph node metastasis or not; and whether lymph node metastasis was positively correlated to molecular markers Ki67、HER-2. 4、The expression of Ki67 was strong positive correlate to HER-2. Conclusion DCE-MRI quantitative parameters of rectal cancer can reflect tumor microvascular perfusion, blood vessels, histopathological differentiation degree, and indirectly measure the biological behavior of rectal cancer tumor tissue. At the same time, the molecular markers Ki67 and HER-2 were promoting role in rectal cancer growth.
