Abstract | 目的:探讨青藤碱对大鼠肾缺血再灌注时核因子(NF-κB)及诱导型一氧化氮合酶(iNOS)水平的影响。
方法: 6~9周龄、体重180~220 g的健康Wistar成年雄性大鼠72只,随机(随机数字表法)分为4组(n=18):对照组(C组)、青藤碱组(SIN组)、肾缺血再灌注组(I/R组)、青藤碱+肾缺血再灌注组(SIN+I/R组)。采用夹闭左肾肾蒂45分钟、再灌注即刻切除右肾的方法制备肾-缺血再灌注损伤模型。再灌注后的6 h、8 h和12 h时,每组随机选取大鼠6只,心脏穿刺采集血标本,用于测定大鼠的血清Cr和BUN浓度;摘取左肾组织,光镜观察肾脏组织病理学变化,非生物素二步免疫组织化学法检测大鼠肾脏NF-κB活性及iNOS的表达。
结果:与C组比较,I/R组和SIN+I/R组大鼠血清Cr和BUN浓度升高,肾组织NF-κB活性升高,iNOS表达上调(P<0.05),SIN组上述指标差异无统计学意义(P>0.05);与I/R组比较,SIN+I/R组大鼠血清Cr和BUN浓度降低,肾组织NF-κB活性降低,iNOS表达下调(P<0.05)。SIN+I/R组肾组织病理学损伤较I/R组减轻。
结论:青藤碱减轻大鼠肾缺血再灌注损伤的机制与抑制NF-κB活性,下调iNOS表达有关。

Other Abstract | Objective: To evaluate the effect of sinomenine on levels of nuclear factor kappa B(NF-κB) and inducible nitric oxide synthase(iNOS )during renal ischemia-reperfusion in rats.
Methods: Seventy-two healthy male Wistar rats,weighing 180-220g,were randomly assigned into 4 groups(18 each group):sham operation group(group C),sham operation combine sinomenine group(group SIN),renal ischemia-reperfusion injury model group(group I/R),sinomenine pretreatment group(group SIN+I/R). The animals were anesthetized with 10% chloral hydrate 350 mg/kg.In group I/R and SIN+I/R,renal ischemia-reperfusion injury model were established,and the left renal pedicles were clipped with atraumatic microclips for 45 min followed by reperfusion;while in group C and SIN,renal pedicle were only exposed but not ligated for 45 min. In group SIN and SIN+I/R,sinomenine 60 mg/kg was injected intraperitoneally at the time of 30 min before reperfusion,while the equal volume of normal saline was given at the corresponding time point in group C and I/R. At the reperfusion time of 6h (T1),8h (T2),12h (T3),6 rats of each group were cut the left renal and killed after gathered the blood samples by cardiac puncture for measurement of serum creatinine(Cr) and blood urea nitrogen(BUN) concentrations. Pathological changes in renal tissues were observed with light and electron microscopes. The expression of NF-κB and iNOS in renal were determined by immune-histochemistry.
Results: Compared with group C,the lever of serum Cr and BUN, expression of NF-κB and iNOS were no significant in group SIN(P﹥0.05);the lever of serum Cr and BUN,expression of NF-κB and iNOS were significantly increased in group I/R and SIN+I/R (P<0.05). Compared with group I/R,the lever of serum Cr and BUN,expression of NF-κB and iNOS were significantly lower (P<0.05),the microscopic examination showed that the renal pathological injury was obviously attenuated in group SIN+I/R.
Conclusion: Sinomenine pretreatment can attenuate renal ischemia-reperfusion injury of rats and its mechanism may be related to inhibition of the activation of NF-κB and the expression of iNOS.
