Abstract"目的:研究幽门螺杆菌(Helicobacter pylori, H.pylori)感染时胃粘膜组织中核因子-κB(NF-κB)p65蛋白、白细胞介素-8(IL-8)及环氧合酶-2(COX-2) mRNA和蛋白的表达及细胞定位,并分析三者之间的关系,探讨NF-κB在H.pylori致病过程中的作用。 方法:采集H.pylori阴性慢性胃炎18例和H.pylori阳性慢性胃炎38例患者的胃粘膜活检标本,采用免疫组织化学法和原位杂交组织化学法检测胃粘膜中NF-κB p65蛋白、IL-8及COX-2的mRNA和蛋白的表达及细胞定位情况。 结果:NF-κBp65、IL-8和COX-2主要表达于胃粘膜上皮细胞,固有层炎性细胞也有不同程度的表达。H.pylori阳性胃粘膜中NF-κB p65蛋白、IL-8及COX-2 mRNA和蛋白的表达较H.pylori阴性组显著增强(5.6±3.3vs3.0±2.5, 3.7±2.6vs1.1±1.3, 3.9±2.9vs1.9±1.6, 3.0±2.4vs1.5±1.1, 2.8±2.3vs1.0±0.7, P<0.01),三者的表达均与胃粘膜炎症程度之间具有显著相关性(P<0.01);H.pylori阳性胃粘膜中细胞核有p65蛋白阳性染色的平均细胞百分率较H.pylori阴性组增高[(38.52±17.32)% vs (11.81±6.04)%];胃粘膜NF-κB p65蛋白表达与IL-8及COX-2 mRNA和蛋白的表达均呈正相关关系(rs分别为0.690,0.709,0.448,0.480,P<0.01)。 结论:H.pylori感染导致胃粘膜NF-κB p65、IL-8及COX-2表达增加,三者均在H.pylori相关性胃炎的发生发展中起作用;H.pylori感染激活胃粘膜NF-κB,使其发生核移位,且胃粘膜中NF-κB表达水平与IL-8及COX-2 mRNA和蛋白表达均呈正相关关系, NF-κB可能通过上调炎性介质IL-8和COX-2的基因转录,参与了H.pylori的致病过程,且在其中占据重要地位。
Other Abstract"Objective To investigate the expression of nuclear factor -κB(NF-κB)p65 protein, interleukin-8(IL-8) and cyclooxygenase-2(COX-2) in H.pylori-infected gastric mucosa and their correlation and explore the role of NF-κB in the pathological process of H.pylori-associated gastritis. Methods Gastric mucosal biopsy specimens were obtained from 38 patients with H.pylori-positive gastritis and 18 H.pylori-negative control subjects. The expression of NF-κB p65 protein and the expressions of mRNA and protein for IL-8 and COX-2 were determined by immunohistochemistry and in situ hybridization. Results NF-κB p65,COX-2 and IL-8 were expressed mainly in gastric epithelial cells, and also in inflammatory cells in lamina propria in variant degrees. The expression of p65 protein and the expressions of mRNA and protein for IL-8 and COX-2 were significantly higher in H.pylori-positive gastric mucosa than that in H.pylori-negative controls (5.6±3.3vs3.0±2.5, 3.7±2.6vs1.1±1.3, 3.9±2.9vs1.9±1.6, 3.0±2.4vs1.5±1.1, 2.8±2.3vs1.0±0.7, P<0.01) and were correlated with histological severity of gastritis(P<0.01). The mean percentage of cell nuclear staining with p65 protein was significantly higher in H.pylori-positive gastric mucosa, compared with H.pylori-negative mucosa[(38.52±17.32)%vs(11.81±6.04)%]. The expression of p65 significantly correlated with that of mRNA and protein for IL-8 and COX-2(Spearmans correlation cofficient respectively was 0.690,0.709,0.448,0.480, P<0.01). Conclusion 1 H.pylori infection increases the expression of NF-κB p65, COX-2 and IL-8 in the gastric mucosa, suggesting that they are involved in the pathological process of H.pylori-associated gastritis. 2 H.pylori infection activates NF-κB in the gastric mucosa and induces nuclear translocation of NF-κB, and the expression of p65 significantly correlated with that of mRNA and protein for IL-8 and COX-2. NF-κB may play an important role in the inflammatory responses to H.pylori by upregulating the gene transcription of IL-8 and COX-2.
