Abstract | 目的:探讨系统性红斑狼疮(SLE)与血浆同型半胱氨酸(Hcy)间的关系,分析影响SLE患者Hcy的代谢因素,以及Hcy在SLE患者发病过程中的重要性和血浆Hcy的检测对于SLE病人的临床价值。
方法:依据1982年美国风湿病协会(ARA)修订的SLE诊断标准,临床收集40例住院SLE患者,以及45例健康献血者和健康体检者为正常对照组。两组均用酶联免疫分析法测定血浆Hcy水平;用时间分辨荧光免疫法测叶酸、维生素B12<下标!>。同时用SPSS 11.5软件包对数据进行统计学处理,进行两组间的比较,并分析Hcy与其它两指标之间的相关性。
结果:1、SLE组血浆Hcy[(17.95±5.99)μmol/L]水平显著高于正常对照组血浆Hcy[(12.14±4.16)]μmol/L水平,两组比较存在显著性差异(P < 0.05);叶酸[(3.91±1.67)nmol/L]、维生素B12<下标!>[(263.98±90.12)pmol/L]均显著低于正常对照组叶酸[(6.15±1.67)nmol/L]、维生素B12<下标!> [(453.28±85.26)pmol/L],两组比较存在显著性差异(P < 0.05)。
2、SLE组患者Hcy与叶酸、维生素B12水平呈负相关,相关系数分别为-0.84、-0.85,P < 0.05。
3、SLE组患者高Hcy血症患病率为70%,正常对照组为20%,两组比较存在显著性差异(P < 0.05)。
4、正常组血浆Hcy浓度与两指标无相关性。
结论:Hcy水平升高与SLE密切相关,SLE患者普遍有高Hcy血症,同时伴有叶酸、维生素B12<下标!>的降低,提示高Hcy血症与SLE及其血管病变显著相关,血浆Hcy的检测对于SLE病人有临床价值。 |
Other Abstract | Objective: To study the relationship between systemic lupus erythematosus(SLE) and plasma homocysteine(Hcy), analyze metabolism factors of Hcy in SLE, investigate the importance of plasma Hcy in SLE and the clinic value of plasma Hcy test in SLE patients.
Methods: 40 SLE patients were enrolled as SLE group and 45 normal subjects as control group. The plasma Hcy levels of these subjects were measured by Enzyme-linked immunosorbent assay. The levels of plasma folate and VitaminB12<下标!> were tested by Time-Resoled Fluoroimmunoassay. The datas were analyzed using the Statistical Package for the Social Sciences (SPSS, version 11.5 for Microsoft Windows). The relations between Hcy and plasma folate, Vitamin B12<下标!> levels were analyzed.
Results: 1. The level of plasma Hcy was significantly higher in SLE group [(17.95±5.99)μmol/L] than in the control group [(12.14±4.16) μmol/L]. The levels of folate, Vitamin B12<下标!> were significantly lower in SLE group[folate(3.91±1.67)nmol/L,Vitamin B12<下标!>(263.98±90.12)pmol/L] than in the control group[folate(6.15±1.67)nmol/L,Vitamin B12<下标!>(453.28±85.26)pmol/L]. There was significantly difference between two groups, P<0.05.
2. The plasma Hcy level in SLE patients was negatively related to folate, Vitamin B12<下标!> levels, the correlation coefficient of folate to Hcy was -0.84, and VitaminB12 to Hcy was -0.85, P<0.05.
3. The prevalence rate of hyperhomocysteinemia was 70% in the SLE patients, and it was 20% in control group. There was significantly difference between two groups, P<0.05.
4. There was no relation between Hcy and these factors in the control group (P>0.05).
Conclusion: Plasma Hcy level was elevated in patients with SLE, meanwhile, there were lower levels of folate and VitB12<下标!>. Plasma Hcy was negatively related to SLE and SLE’s blood vessels diseases. There was clinical significance in the detection of plasma Hcy in SLE patients. |
