新型造影剂在犬急性胃黏膜病变超声造影的实验研究

Abstract目的:急性胃黏膜病变(AGML)是急诊科最常见的疾病之一,是创伤后最为常见的内脏并发症之一,对患者有潜在的致命性威胁。目前临床对急危重症患者合并胃肠道疾病缺乏有效的诊断手段。尽管有传统的X线钡餐和胃肠内镜检查,但这些检查仍然受到一定限制。本实验运用自行研制的新型胃超声造影剂,研究造影剂在正常大鼠和犬的胃显影,研究急性胃黏膜损伤的超声造影声学表现。 方法:本实验采用健康成年SD大鼠42 只,随机分为两组。1)组为正常对照组,注入胃窗造影剂;2)组注入新型造影剂。根据结果我们选出造影效果最好的新型胃超声造影剂。成年杂种犬16只,随机分为正常对照组和实验组。实验组建立急性胃黏膜损伤模型。两组犬经胃管注入新型胃肠超声造影剂后超声观察检测,分别记录超声声像图。然后开腹取胃,切取胃壁组织,制片染色,光镜下观察,并与超声声像图对照。 结果:新型造影剂组与胃窗对照组比较(P>0.05)。两种超声造影剂显影效果比较无明显差异。超声声像图上,对照组犬胃腔充盈良好,胃壁结构层次清晰,即显示“强-弱-强-弱-强”五层回声;与对照组相比,实验组犬胃窦部胃壁明显增厚,第1、第2及第3层混为一层,且回声增强。应用胃肠超声造影检查可清晰显示正常犬及急性胃黏膜损伤模型犬胃壁的结构层次。 结论:超声造影对急性胃黏膜损伤有一定的诊断价值;提示在临床超声造影检查做为诊断胃肠疾病的主要手段具有十分重要的意义。
Other AbstractObjective Acute gastric mucosa lesion is one of the common disease in Emergency Department, and integral organs complication after wounding ,which could threaten patients lives.. In recent years, there is no effective diagnose method in clinic about severe patients to put together gastrointestinal trace disease. Though we could have traditionary detection method on morphology of stomach intestine including barium meal and Endoscopy of stomach intestine, there are surely confined frequent. This experiment makes use of a new type gastric ultrasound contrast agent which is own invented, investigating to develop an imaging stomachs of SD rats and dogs by gastric ultrasound contrast. To investigate the appearance of ultrasound contrast sonogram in acute gastric mucosa injury.. Methods Forty-two Sprague-Dawley (SD) rats were randomly allocated into two groups. The first group is the control group, each SD rats was poured in the Wei-chuang UCA through gastric tubes. The second group each SD rats was poured in the new type UCA through gastric tubes. We picked out the best new type gastric UCA through these experiments. 16 adult hybird dogs were randomized into a normal control group and an experimental group. Acute gastric mucosa injury models were made in the experimental group. The gastric ultrasound sonogram was observed and recorded after each dog was poured in the new type UCA through gastric tubes. The stomachs of two groups dogs were cut out, and their stomach tissues were cut into slices, dyed, and observed under microscope which would be compared with sonogram. Results The research shows: the new type UCA group compared with Wei-chuang UCA group(P>0.05),there were no obvious difference. The ultrasound sonogram showed that the stomachs of dogs in the control group were filled with UCA well, the structure of gastric wall could display clearly on 5 layers -“strong-weak- strong-weak-strong”; Comparing with the control group, the sonograms of the experimental gastric antrum wall in dog displayed much thicker, and the first, second and three-tier were mixed each other resulting in more strengthener echo. The structure of gastric wall in the normal and acute gastric mucosa injury of dogs could display clearly by gastrointestinal contrast ultrasound.
