Abstract | 目的 评价多重PCR在快速诊断呼吸机相关性气管-支气管炎和肺炎常见病原菌中的临床作用和价值。方法 选择常见病原体特异性基因序列,即金黄色葡萄球菌的mecA、FemA基因、铜绿假单胞菌的脂蛋白I基因(OprI)、大肠埃希氏杆菌的PhoA基因、肺炎克雷白杆菌的mdh基因以及鲍曼不动杆菌的OXA-51型酶基因(blaOXA-51-like)设计并合成6对引物,通过经典酚氯仿提取痰液标本中的DNA,扩增病原体DNA后电泳并观察其结果,同时给予普通PCR和分离培养作为对照。结果 (1) 多重PCR与普通PCR检测比较:对痰标本同时进行多重PCR和普通PCR检测,两者均检测出金黄色葡萄球菌20例,鲍曼不动杆菌20例,大肠埃希氏杆菌10例,绿脓假单胞菌20例,肺炎克雷白杆菌15例,其中有15份标本为以上任何2种菌的混合感染。多重PCR与普通PCR的符合率达100%。结论 M-PCR技术能够高通量、快速检测诊断病原体,具有较好的敏感性和特异性。可能为VAT或VAP的临床诊断提供一个新的快速检测手段,也为临床治疗提供更为准确的参考依据。 |
Other Abstract | Objective To observe the rapid diagnosis value of multiplex PCR in patients with Ventilator-Associated Tracheobronchitis and Ventilator-Associated Pneumonia.Methods Select a common pathogen-specific gene sequences, Staphylococcus aureus mecA, femA genes、Pseudomonas aeruginosa lipoprotein I gene (OprI)、Escherichia coli PhoA genes、Kpneumoniae mdh genes、Acinetobacter baumannii OXA-51 type enzyme gene, Through the DNA in the sputum of classic phenol-chloroform extraction, the amplified pathogen DNA electrophoresis and observe the results, while giving the PCR and separation of culture as a control. Conclusion M-PCR is a high- flux, rapid detection and diagnosis of pathogens, with better sensitivity and specificity。May provide a new means of rapid detection of VAT or VAP clinical diagnosis, provide a more accurate reference for clinical treatment. |
