Abstract目的 探讨舍曲林抗白念珠菌活性及与常规抗真菌药相互作用,为研究舍曲林抗真菌机制打下基础。 方法 1.对分离自临床上的132株念珠菌进行菌种鉴定。 2.采用美国临床实验室标准化研究所(CLSI)制定M27-A中的微量稀释法检测舍曲林对103株白念珠菌的最小抑菌浓度(MIC)。 3.采用棋盘微量稀释法对18株白念珠菌进行舍曲林和常规抗真菌药物的体外联合的抑菌实验。 结果 1.分离的132株念珠菌经鉴定证实为白念珠菌103株、热带念珠菌7株、近平滑念珠菌9株、克柔念珠菌2株、光滑念珠菌10株及季也蒙念珠菌1株。 2.舍曲林对白念珠菌有抗菌活性,其MIC值为3.715~59.375μg/ml。 3.舍曲林与氟康唑联用时对氟康唑敏感株及耐药株表现出协同作用;两者之间的部分抑菌浓度指数(FICI)差异无显著性(P=0.285)。 4.舍曲林与伊曲康唑、氟康唑和两性霉素B联合应用时FICI值分别为0.3~1.0、0.3~1.5及0.2~1.0,这种相互作用无显著性差异(P=0.738)。 结论 1.从临床念珠菌感染患者标本中分离出的菌株仍以白念珠菌为主。 2.舍曲林对白念珠菌具有一定的抗菌作用。 3.舍曲林与氟康唑联合应用时主要表现为协同作用,在氟康唑敏感株及耐药株之间无显著性差异。 4.舍曲林与常规抗真菌药物联用时对白念珠菌主要表现为协同作用,相互作用无显著性差异。
Other AbstractObjective To determine the anti-Candida albicans activity of sertraline in vitro and the combined effect of sertraline with itraconazole, fluconazole or amphotericin B,so as to set up the foundation for clarifying antifungal mechanism. Methods 1. One hundred and thirty-two Candida strains were isolated from varieties of clinical specimens and then identified. 2. The minimum inhibitory concentration(MIC)of sertraline to 103 clinically isolated Candida albicans were tested by CLSI-M27-A microdilution method,meanwhile fluconazole and amphotericin B used as quality control drugs. 3. The checkerboard microdilution method was applied to assay the combination effect of sertraline with fluconazole,amphotericin B or itraconazole for 18 strains of Candida albicans. Results 1. Of total 132 isolates,the results of identification are as follows:103 strains of C.albicans,7 C.krusei,9 C.parapsilosis,2 C.tropicalis,10 C.glabrata,and 1 C.guilliermondii. 2. Sertraline has inhibitory effect on Candida albicans tested,and its range of MICs are 3.715~59.375μg/ml. 3. Synergistic and no interaction are observed obviously,without antagonism in the combination effect of sertraline with fluconazole.The mean fractional inhibitory concentration index (FICI) between fluconazole-susceptible and -resistant Candidas albicans is no significant difference (P=0.285) . 4.Sertraline have influence on itraconazole,amphotericin B or fluconazole against Candida albicans,and the range of FICIs are 0.3~1.0,0.3~1.5,and 0.2~1.0,respectively,meanwhile there are no significant difference on the influence(P=0.738). Conclusions 1. Most of the Candida stains isolated from clinical patients of Candida infection are Candida albicans. 2. Sertraline has antifungal effects on medically important Candida albicans. 3. The interactions between sertraline and fluconazole are mainly synergistic effects,and this interaction in fluconazole-susceptible and -resistant Candidas albicans is no significant difference. 4.Synergistic effects are main interactions between sertraline with fluconazole,amphotericin B or itraconazole against Candida albicans,and the interactions is no significant difference.
