Abstract目的: CD24是一种最近在肿瘤生物学中引起广泛关注的黏蛋白样黏附分子,参与细胞之间的黏附和肿瘤细胞的扩散转移,CD24是一种低分子量高度糖基化的蛋白质黏附分子,由27个氨基酸组成,通过糖基化磷酯酰肌醇黏附在细胞膜上,具有多种功能性的糖基化模式。在小鼠,CD24主要表达在造血细胞、胸腺细胞、脑、胰腺的上皮细胞,人红细胞和胸腺细胞并不表达CD24,而在血液系统肿瘤和一些器官的实体瘤表面可见CD24高表达。最近研究表明,CD24可高表达于B细胞淋巴瘤、肾细胞癌、麦克尔细胞癌、神经源性肿瘤、胰腺癌、胃癌、大肠癌、乳腺癌等多种肿瘤细胞表面,且与肿瘤的侵袭转移、复发及预后密切相关,因此CD24已成为一个某些恶性肿瘤的侵袭、转移标志和预后的指标。探讨CD24和血管内皮生长因子(VEGF)在膀胱尿路上皮癌中的表达及与肿瘤的临床特征的关系。 方法: 采用免疫组织化学SP法检测了120例(80例经尿道切除标本,40例根治性膀胱切除标本)膀胱尿路上皮癌组织中CD24、VEGF的表达,根据VEGF的染色情况计算出膀胱尿路上皮癌组织的微血管密度(MVD)。 结果: 在正常尿路上皮中,CD24局限于细胞浆的极弱表达。膀胱尿路上皮癌组织中CD24的阳性表达率明显高于正常膀胱上皮。高的肿瘤分级和高的pTNM分期、淋巴结转移的癌组织中CD24的阳性表达率显著高于低的肿瘤分级和低的pTNM分期、无淋巴结转移的癌组织(P<0.05)。膀胱尿路上皮癌中VEGF、MVD表达高于正常膀胱上皮,与高的肿瘤分期、浸润及淋巴结的转移密切相关。即膀胱尿路上皮癌中VEGF、MVD随其分期的增高而增加;有浸润和淋巴结转移者分别明显高于无浸润和淋巴结转移者,膀胱尿路上皮癌组织中CD24阳性表达率与MVD呈正相关(r=0.243,P<0.05)。 结论: CD24在膀胱尿路上皮癌中表达明显上调和膀胱尿路上皮癌临床分期,病理分级呈正相关,与肿瘤的浸润、转移、血管形成密切相关。VEGF、MVD在膀胱尿路上皮癌中与肿瘤的分期、浸润及淋巴结的转移密切相关,膀胱尿路上皮癌中VEGF、MVD随其分期的增高而增加,CD24与VEGF、MVD的表达有相关性,CD24和VEGF在膀胱上肿瘤的血管形成过程中可能具有协同作用。 关键词: 膀胱尿路上皮癌;CD24;血管内皮生长因子(VEGF); 微血管密度(MVD);浸润转移
Other AbstractObjective: CD24 is a molecule that recently has raised considerable attention in tumour biology. It is involved in cell adhesion and metastatic tumour spread .In the mouse, CD24 was expressed in haematopoietic cell, thymocytes, brain and pancreas. In contrast to the mouse , CD24 was not expressed on human erythrocyte or thymocytes, but CD24 was high expressed in hematological malignancies as well as in several types of solid tumors. Recently studies revealed that high rates of CD24 expression detected in a variety of the most common tumours, such as B cell lymphoma, renal cancer, Merkel cell cancer, neura cell lineage tumors, pancreatic cancer, gastric cancer, colorectal carcinoma, breast cancer and so on. Moreover, high rates of CD24 expression was significantly associated with invasion metastasis ,relapse and prognosis, CD24 has been a marker of invasion metastasis ,relapse and prognosis in some malignant tumour.To investigation the clinical signifiance of CD24 and vascular endothelial growth factor (VEGF) expression in urothelial carcinoma of the bladder. Methods:To detect expression of CD24 and VEGF in 120 specimens(80 transurethral resection,40 radical cystectomy) of bladder urothelial carcinoma and 10 specimens normal bladder tissues by SP immunohistochemistry, Microvessel density(MVD) was figured out according to the positive staining for VEGF. Results:In normal urothelium,CD24 was localized to the cytoplasm of cell with very low intensity,and CD24 expression was higher in bladder urothelial carcinoma than that in the normal urothelium, the CD24 higher expression was significantly correlated with high tumor grade, high pTNM stage, lymph node metastasis(P<0.05).The VEGF、MVD higher expression were significantly correlated with high tumor grade, invasion, lymph node metastasis.The expression of VEGF、MVD were increased with high pTNM stage. VEGF、MVD expression was higher in bladder urothelial carcinoma of invasion, lymph node metastasis than that of noninvasion,no lymph node metastasis,the expression of CD24 has positive correlation with the MVD(r=0.243,P<0.05). Conclusions: CD24 expression is obviously up-regulated in urothelial carcinoma was statistically significant, the expression of CD24 has positive correlation with the grade and stage of tumor,and it is correlated with the tumor invasion,metastasis and angiogenesis.The VEGF、MVD higher expression were significantly correlated with high tumor grade, invasion, lymph no...
