Abstract目的 观察TGF-β1-胰岛素复合凝胶对深Ⅱ度烫伤后的糖尿病大鼠创面愈合的影响。方法 以卡波姆980为基质,分别加入TGF-β1 2 ug、胰岛素1600 U,制备空白凝胶、胰岛素凝胶、TGF-β1凝胶、TGF-β1-胰岛素凝胶。选取75只重量在200g至290g之间的大鼠,将选取的大鼠随机分为A、B、C、D、E五组,A组:正常烫伤大鼠对照组、B组:糖尿病烫伤大鼠对照组、C 组:胰岛素凝胶烫伤大鼠组、D组:局部TGF-β1凝胶烫伤大鼠组、E组:局部TGF-β1-胰岛素复合凝胶烫伤大鼠组,每组15只。空白组给予腹腔注射柠檬酸钠缓冲液,B、C、D、E组则给予腹腔注射STZ,55 mg/kg。将各组大鼠烫伤成深Ⅱ°模型。并涂抹对应凝胶。在5个时间点观测其皮肤创面的愈合情况(3、7、11、15、21),计算愈合率,并在各时间点进行组织观察及免疫组织化学染色切片观察。结果 实验所制备的各组凝胶均无色透明。实验中所用的各组大鼠未发现死亡。烫伤后前三天观察发现各组大鼠创面无明显缩小,在实验第7d、11d、14d、21d各观察点观察可发现E组大鼠创面愈合率均为最高。而B组均为最低,与A、C、D、E四组相比较以及E组、B组之间相互比较,差异均有统计学意义(P < 0.05)。观察发现在5个时间点中,TGF-β1-胰岛素组的血管数量及Ⅰ型胶原,III型胶原含量高于其它组。各组微血管数量、Ⅰ型胶原蛋白,III型胶原蛋白阳性率均于3d开始表达,且随时间延长阳性细胞逐渐增多;各时间点中E组大鼠微血管密度及Ⅰ型胶原蛋白,III型胶原蛋白含量最高,而B组大鼠微血管密度及Ⅰ型胶原蛋白,III型胶原蛋白含量则最少,跟其它组大鼠相比较差异有统计学意义(P < 0.05),TGF-β1凝胶、TGF-β1-胰岛素两组大鼠间相互比较差异有统计学意义(P < 0.05)。结论 1.糖尿病大鼠深II度烫伤模型有助于对糖尿病患者难愈性创面机制的研究,外用TGF-β1-胰岛素凝胶可促进深II度烫伤糖尿病大鼠创面的愈合。2.外用TGF-β1-胰岛素凝胶可促进细胞的纤维化及相关细胞功能,促进创面组织的修复,提高创面愈合率。同时还能够促进组织中胶原的合成,肉芽组织的生长以及毛细血管的形成。
Other AbstractObjective Observe the effects of TGF-beta1 insulin composite gel on deep second degree scald wound healing in diabetic rats.Methods Carbomer 980 gel as the martix, add TGF-beta1 2 ug and insulin 1600 U into the Carbomer 980 gel so that to get the blank gel、insulin gel、TGF-β1 gel and TGF-β1-insulin composite gel. Totally 75 male clean SD rats were fed with well ,weighing between 200g to 290g,The diabetic rats model was designed by abdominal injection of STZ (55 mg/kg),and then all the rates were burned became the deep second degree scald wound. The 75 rates were randomly divided into five groups(A、B、C、D、E),free control (Con) group(n=15) Diabetes group(n=15, Insulin group(n=15),TGF-β1 gel group (n=15),TGF-β1- insulin composite gel group (n=15).Different kinds of gel were daubed on the wound of the rates to observe the effect of the skin healing rate, on the 3th、7th、11th、15th 、and 21th day, healing rate is calculated at each time point,rats in each group were sacrificed three,at each point day,and then full-thickness skin samples were taken for histological and immunohistochemistry staining.Result All the carbomer gel we got were Colorless and transparent .During the period of the experiment, all the rates were survived, there is a small difference in the five groups in the first three days, at the 7th、11th、15th and 21th day ,we can see that, the healing rare in the TGF-β1 gel group was the highest (76.69%) , however, the Diabetes group was the minimum, which compared with the other four groups (P<0.05),In the five point time, we find that, the TGF-β1- insulin composite gel group with the largest number of capillaries, and with the highest densities of Ⅰand III collagen, but the Diabetes group was the minimum, In the five groups we can observe that, the number of capillaries、the densities of Ⅰand III collagen, all of them started to increase from the third day, and arrived to the top at the 15th day, the Diabetes group compared with the other four groups has the obvious advantages (P<0.05). TGF-β1 gel group compared with TGF-β1- insulin composite gel group was significantly (P <0.05).Conclusion 1.Diabetes rates model is helpful to discuss the mechanism of the Diabetes, and TGF-β1- insulin composite gel is good for the chronic and refractory wound.2.TGF-β1- insulin composite gel can promote cell fibrosis and promote functions of cells, on one hand which can promote wound repair tissue, improve wound healing rate....
