Abstract目的:探索性研究剪切波组织定量(Elastography Point Quantification,Elast PQ)技术评价非酒精性单纯性脂肪肝(Nonalcoholic simple fatty liver,NAFL)的可行性及适用性,及其与代谢指标的相关性,综合评估Elast PQ技术联合代谢指标对非酒精性单纯性脂肪肝的临床诊断价值。 方法:应用Philips IU Elite 超声诊断仪配备的Elast PQ软件技术对39例健康体检者与46例NAFL患者行肝脏硬度检测,依据ROC曲线判定Elast PQ技术诊断NAFL的诊断界值;应用全自动生化分析仪采集血生化指标中的丙氨酸转移酶(ALT)、甘油三脂(TG)、高密度脂蛋白胆固醇(HDL—C)、同型半胱氨酸(Hcy)等生化指标及部分常见基线参数,综合分析各检测指标之间的相关性以及各指标与NAFL的相关性。 结果:肝脏Elast PQ值在正常人群与NAFL患者中存在明显统计差异(t=2.227,P=0.031);通过ROC曲线计算得:0.880±0.020为曲线下最大面积,标准误为0.020,Elast PQ技术对NAFL的评价有临床统计意义,95%的置信区间(0.817、0.936)。Elast PQ值为3.61 kpa时,相应的特异度及敏感度最高,分别为69%、89%。WHR、BMI、TG、CHO、LDL-C在两组中比较,统计学意义明 显(P<0.050),并且肝脏Elast PQ值在NAFL组和正常组中分别与AST/ALT、WHR有正相关性(r1=0.564,P1=0.018;r2=0.566,P2=0.044)。 结论:Elast PQ技术联合代谢指标对NAFL的定量评价是可行的、适用的,具有广阔的临床诊疗前景。
Other AbstractObjective: To preliminary explore the application of shear wave quantitative technique evaluation organization Nonalcoholic simple fatty liver the feasibility and its correlation with metabolism related indicators, comprehensive evaluation Elast PQ technology combined metabolic index for clinicaldiagnostic value of NAFL. Method: Application of Philips IU Elite ultrasonic diagnostic instrument Elast PQ software technology for 39cases of normal physical examination and 46cases of nonalcoholic simple fatty liver patients liver elastic hardness testing, determined by the ROC curve Elast PQ technology the diagnostic boundary value in the diagnosis of NAFL. Application of automatic biochemical analyzer collecting pieces liver function and biochemical indexes of metabolism and some common baseline parameters, comprehensive analysis of the correlation between the detection index and the index and the correlation of NAFL. Rusult: Normal people and Nonalcoholic simple fatty liver is significantly difference in Elast PQ value (t=2.227,P=0.031); By statistical calculation to large area under the ROC curve for 0.88 ± 0.02, the standard error of the area for 0.02, Elast PQ for assessing nonalcoholic simple fatty liver was statistically significant, 95% confidence interal (0.817, 0.936). Elast PQ value is 3.16kpa, the sensitivity of corresponding highest and specific degree, 89 % 、69% respectively. The case group and normal group of WHR, BMI, TG, CHO, LDL - C, Elast PQ value comparison have significant differences, there are statistical significance (P < 0.050), in NAFL group and normal group of Elast PQ value respectively have correlation with AST/ALT, WHR (r1=0.564,P1=0.018;r2=0.566,P2=0.044). Conclusions:Elast PQ technology combined the metabolic index to the diagnosis of nonalcoholic simple fatty liver is feasible, and it has a broad prospect in clinical diagnosis and treatment.
