Abstract目的:探讨PGI2/ TxA2受体(IP/TP)与环氧合酶2cyclooxygenase-2,COX-2)在肝脏移植缺血再灌注损伤(ischemical reperfusion injury,IRI)中的机制及相互作用。方法:雄性SD大鼠随机分为3组:正常对照组(E1)、肝脏移植组(E2)、特异性COX-2抑制剂+肝脏移植组(E3),手术后3h、6h、12h、24h留取标本检测。RT-PCR检测肝组织中IPmRNA、TPmRNA及COX-2mRNA的表达;免疫组化法检测COX-2在组织中的定位及表达; HE染色确定组织损伤程度,血清ALT、AST检 测肝酶变化。结果:E2组COX-2免疫组化与E1组相比表达明显增加,主要分布于汇管区肝窦内皮细胞、肝细胞及巨噬细胞胞内,E3组COX-2表达较E2组明显降低;E2组中IPmRNA、TPmRNA、COX-2mRNA表达水平均比E1组明显升高(P<0.05),IP/TP比值升高(P<0.05), E3组IP和TP较E2组均明显降低(P<0.05),IP/TP比值降低(P<0.05);HE染色见E2组肝损伤明显,E3组肝损害较E2组明显减轻;血清中AST、ALT在E2组明显增高(P<0.05),E3组较E2组明显下降(P<0.05),以上指标均在再灌注6小时时达峰值。结论:IP/TP的平衡在肝脏移植缺血再灌注损伤中具有重要作用,抑制COX-2的表达可通过改善IP/TP 的失衡从而减轻肝移植缺血再灌注损伤。
Other AbstractObjective: To study the expression of PGI2/TxA2 receptor (IP/TP)and cyclooxygenase-2 (COX-2) in rat sufferring from ischemia-reperfusion injury after liver transplantation and its mechanism.Method: Rats were randomly divided into three groups: control group(E1),orthotropic liver transplantation group(E2)and Nimesulide associated with orthotropic liver transplantation group(E3).The samples were collected at 0,3,6,12 and 24h after operationv, of which the expression of COX-2 mRNA,IPmRNA and TPmRNA was detected by RT-PCR, expression and location of COX-2 detected with immunohistochemistry , extent of liver injury classified with Hematoxylin Eosin staining, and serum ALT,AST detected to tell change of liver enzyme.Result:immunohistochemistry in E2 Group showed that COX-2 expression was significantly increased,mainly distributed in the district of liver sinusoidal endothelial cells, whereas in liver cells and macrophage cells, COX-2 expression in E3 group was significantly decreased. Expression of IPmRNA,TPmRNA,COX-2mRNA in the E2 group was significantly increased than that of E1 group (P <0.05), and ratios of IP / TP increased (P <0.05), TP and IP in E3 group were significantly lower (P <0.05),and the ratios was reduced than E2 group(P <0.05); From HE staining in E2 group liver injury can be seen Obviously, and liver damage in E3 group reduced than E2 significantly. Serum AST and ALT in the E2 group was higher than that of E3 group of which was decreased notely (P <0.05).The above phenomena were up to the most in six hours of reperfusion.Conclusion : The balance of IP/TP play a great role in the ischemia reperfusion injury of liver transplantation and liver transplantation IRI. Transplantation IRI can be ameliorated whih changing imbalance of IP/TP in succession with inhibition of liver COX-2.
