Abstract目的 应用免疫组化的方法,对人脑胶质瘤中MGMT及MRP5表达情况进行检测,分析二者的相关性及意义,为临床个体化化疗提供参考。 方法 选取兰大二院神经外科2012-01-2013-08手术切除,经病理证实为脑胶质瘤的存档标本57例, 其中WHOⅠ10例,WHOⅡ18例,WHOⅢ15例,WHOⅣ14例。应用免疫组化(IHC)的方法检测肿瘤组织中MGMT和MRP5的表达情况,对所得资料进行统计学处理,并分析其相关性以及与患者性别、年龄之间的关系。 结果1.在57例胶质瘤标本中,MGMT的阳性率为68%,在Ⅰ,Ⅱ,Ⅲ,Ⅳ级人脑胶质瘤中的阳性率分别为30%,66%,73%,92%。统计检验结果显示MGMT 的阳性表达率随着级别的升高而上调,呈现正相关,有统计学意义(P﹤0.05)。 2.在57例胶质瘤标本中,MRP5的阳性率为60%,在Ⅰ,Ⅱ,Ⅲ,Ⅳ级人脑胶质瘤中的阳性率分别为30%,56%,60%,86%。统计检验结果示:MRP5 的阳性表达率随着级别的升高而上调,呈正相关相关,有统计学意义(P﹤0.05)。 3.MGMT和MRP5在不同级别人脑胶质瘤中的表达与患者性别,年龄无统计学意义(P﹥0.05)。MGMT在低等级(Ⅰ-Ⅱ级)和高等级(Ⅲ-Ⅳ级)胶质瘤租织中的阳性分别为53.6%和82.8%,差别有统计学意义(p<0.05)。MRP5在低等级(Ⅰ-Ⅱ级)和高等级(Ⅲ-Ⅳ级)胶质瘤租织中的阳性分别为53.6%和72.4%,差别有统计学意义(p<0.05)。 4.MGMT与MRP5在不同级别胶质瘤中的表达无明显相关性 (P﹥0.05)。 结论1.胶质瘤中MGMT和MRP5表达与肿瘤病理级别有关呈正相关 ,与患者性别,年龄没有关系。 2.MGMT与MRP5在不同级别胶质瘤中的表达无明显相关性,提示二者耐药机制没有协同的作用。 3.检测脑胶质瘤中MGMT和MRP5的表达,有助于对胶质瘤预后和耐药性做出判断,同时为临床胶质瘤的个体化化疗提供论理基础。
Other AbstractObject To determine the expression of MGMT and MRP5 in human brain glimas by immunohistochemical assay and analyze their correlation. Methods Fifty-seven specimen of glimas from the patients under surgery in the department of neurosurgery of the second hospital of Lanzhou University from January, 2012 to August, 2013. Among all the 57 glioma cases, 10 were WHO grade Ⅰ,18 grdae Ⅱ,15 grade Ⅲ,14 grade Ⅳ. The expression of MGMT and MRP5 in all the specimen were determined by immunohistochemical assay. The correlation between the gender and age of patients was analyzed. The data were processed by SPSS. Results 1. Of all the 57 specimen, 68% indicated positive expression of MGMT, and the MGMT-positive expression of grade I, II, II, IV specimen was 30%,66%,73%,92% respectively. The data was significantly different (P<0.05). 2. Of all the 57 specimen,60% indicated positive expression of MRP5, and the MRP5-positive expression of grade I, II, II, IV specimen was 30 %,56 %,60%,86% respectively. The data was significantly different (P<0.05). 3.The positive expression rate of MGMT and MRP5 has Statistical significance in different pathological grades(P<0.05). The gender and age has no effect on the expression of MGMT and MRP5. The positive expression rate of MGMT at low level (Ⅰ-Ⅱ grade ) and high level (Ⅲ-Ⅳ grade ) glioma tissues is 53.6% and 82.8% respectively , differences are statistically significant (p<0.05). The positive expression rate of MRP5at low level (Ⅰ-Ⅱ grade ) and high level (Ⅲ-Ⅳ grade ) glioma tissues is 53.6% and 72.4% respectively , differences are statistically significant (p<0.05). Conclusions 1. The positive expression rate of MGMT and MRP5 in human glioma correlates with pathological grades of glioma, but doesn’t correlate with the gender and age of patients 2.There is no significant correlation between the positive expression of MGMT and MRP5. 3.The determination of the expression of MGMT and MRP5 in human cells is helpful for prognosis and drug resistance. Meanwhile, it also could provide theory support for individual chemotherapy for glioma in clinic.
