Abstract | 目的:
1. 探讨多层螺旋CT灌注成像在肺局限性病灶鉴别诊断中的价值;
2. 通过CT灌注参数值(BV、BF、MTT、PS)与MVD、EGFR的相关性研究,为肺癌血管生成的活体影像学评价提供依据;
3. 探讨低剂量CT灌注成像的临床应用价值。
材料和方法:
1. 研究对象:2012年7月1日 至2014年3月1日期间就诊于兰州大学第二医院,并行多层螺旋CT(Multislice Computerized Tomography MSCT)灌注成像的83例患者,其中肺癌57例、炎性肉芽肿11例、其他良性病灶15例。
2. 检查方法:
⑴CT灌注成像:所有病例使用GE宝石CT扫描仪,在获得患者知情同意并签署知情同意书后,首先进行常规胸部CT平扫,然后以4.5ml/s速度注射碘对比剂(370mgl/m1)40ml,对病灶行连续动态扫描,在ADW4.5工作站上用body perfusion分析软件处理数据,计算出相关灌注参数。
⑵病理学检查:取57例肺癌手术切除标本及10例癌旁正常组织,行免疫组化法检测MVD、EGFR并计数。
3. 统计学分析:
⑴不同类型肺局限性病灶各灌注参数值的差异性比较;
⑵CT灌注参数和肺癌MVD、EGFR相关性分析;
⑶常规剂量组和低剂量组的BF值、BV值差异;
应用 SPSS17.0 统计学分析软件进行统计学分析,在单因素分析中,计数资料组间比较采用卡方检验,用 Spearman 等级相关分析法来分析两变量间的相关性;检验水准p﹤0.05。 |
Other Abstract | Objective:
1. To investigate the value of multi-slice spiral perfusion CT for focal pulmonary
lesions.
2. By investigating the correlation between lung cancer perfusion parameters and MVD、EGFR, to provide the basis for angiogenesis evaluation of lung cancer in vivo with MSCT.
3. To investigate the clinical value of low-dose CT perfusion imaging.
Materials and methods:
1. Study object: Between July 1, 2012 and March 1, 2014, 83 patients who undertaken treatment in Lanzhou University Second Hospital, and received multislice spiral CT (Multislice Computerized Tomography MSCT) perfusion examination, including 57 cases of lung cancer, 11 cases of inflammatory granuloma and 15 cases of other benign lesions.
2. Examination method:
⑴CT perfusion examination: All patients first underwent unenhanced CT scanning by Discovery CT 750 HD scanner. With the patients informed consent and signed written informed consent. First, conventional thoracic CT scan was obtained. Then, dynamic scans were performed at the location of the lesions after injecting 40ml nonionic contrast administration(370mgl/m1)at the speed of 4.5ml/s.To calculate the relevant perfusion parameters with body perfusion analysis software processing data on ADW4.5 workstation.
⑵Pathological examination: Immunohistochemical examination was performed on 57 cases of lung cancer resection specimens and 10 cases of normal tissue adjacent to carcinoma for detecting MVD, EGFR and counting.
3. Statistical analysis:
⑴To compare CT perfusion parameter values among different lesions.
⑵To evaluate correlation between CT perfusion parameter values and MVD, EGFR expression of lung cancer.
⑶To compare the difference in BF and BV between conventional group and low-dose group.
All results were caculated with a statistic software(SPSS 17.0). In the single factor analysis, chi-square test was used on count data, Spearman analysis was used on correlative research.P﹤0.05 was considered to be statistically significance. |
