Abstract目的:通过检测环氧化酶-2(COX-2)在宫颈上皮内瘤样病变(CIN)、宫颈癌的表达以及血清中前列腺素(PGs: PGE2、TXB2、6-k-PGF1α)含量的变化,探讨COX-2、PGs与宫颈癌前病变、宫颈癌的关系以及它们之间的相关性。 方法:用逆转录聚合酶链反应(RT-PCR)、免疫组化(S-P)法检测 COX-2 mRNA、COX-2蛋白分别在20例正常宫颈组织、20例慢性宫颈炎性组织(宫颈糜烂)、20例CIN组织和40例宫颈癌组织的表达;用放射免疫(RIA)法测量各组患者血清中PGs的含量,并分析其与临床特征的关系。 结果:⑴:COX-2 mRNA在正常宫颈组织无表达,在炎性及CIN组织表达增加(阳性率分别为72%、63%),与正常组对照差异均有显著性(P<0.001),炎性组与CIN组间对照差异无显著性(P>0.05),在宫颈癌组织则明显增加(阳性率为90%),与宫颈炎组以及CIN组相比差异均有显著性(P<0.001),不同临床分期、分级、病理分型、不同癌灶大小间COX-2 mRNA表达无显著性差异(P>0.05);⑵:COX-2蛋白在正常宫颈组织无表达,在宫颈炎组织和CIN组织表达阳性率分别为40%、45%,两组间对照差异无显著性(P>0.05),在宫颈癌组高表达,阳性率为89%,与宫颈癌的临床病理特征无关;(3):COX-2蛋白在癌灶浸润区间质的表达比在癌细胞的表达更强;(4):PGs含量在炎性及CIN组血清中较正常组增加(P<0.05),在宫颈癌组增加更明显(P<0.001),且与COX-2的表达呈正相关。 结论: COX-2、PGs在宫颈炎性及CIN组表达开始升高,在癌性组明显升高,揭示COX-2可能是癌症发生的早期基因,即早就通过PGs促进癌的发生、发展,参与宫颈炎→CIN→宫颈癌的演变。监测血清中PGs的含量可能会成为预测宫颈癌发生的一种方法。
Other AbstractObjective: To study the expression of cyclooxygenase-2 (COX-2)、prostaglandins (PGs) in cervical tissues of differential pathological types,especially in cervical intraepithelial neoplasia (CIN) and cervical carcinoma, and their possible relationship in carcinogenesis. Methods: Tissue blocks and blood samples from 20 normal、inflammatory、CIN and 40 carcinous cervix respectively. COX-2 mRNA was examined by reverse transcription-polymerse chain reaction (RT-PCR). COX-2 protein was measured using immunohistochemistry. PGs were detected by radioimmunoassay (RIA). Result: ①:COX-2 was not expressed in normal cervix, and the expression was increased in inflammatory and CIN (P<0.001), highly increased in cervical carcinoma(P<0.001).②:There was no different in different tissues of cervical carcinoma.③: The expression in cancer was observed weaker than that in invasive interstitium. ④:The quantity of PGs was increased in serum of inflammatry and CIN than that in serum of normal cervix, highly increased in carcinoma group. Conclusion: Current study indicated that COX-2 was an immediate-early gene involved in carcinogenesis in precancerose lesion of cervical carcinoma , and acceletated the progress of tumor.
