重组人红细胞生成素对早产儿脑白质发育神经保护机制分析

Abstract目的分析重组人红细胞生成素(rhEPO)对早产儿脑白质发育神经保护的影响。方法回顾性分析接受治疗的早产儿160例,依据患儿住院号尾号单双数将其分成观察组(80例)与对照组(80例),观察组在出生72 h内皮下注射剂量为300 IU/kg的rhEPO,对照组则皮下注射0.2 mL的生理盐水,患儿镇静入睡以后行磁共振弥散张量成像(DTI)、DWI和头颅MRI检测,分析患儿临床资料、母孕期状况和胎儿相关并发症状况。结果两组患儿新生儿胆汁淤积症、支气管肺发育不良(BPD)、败血症、新生儿坏死性小肠结肠炎(NEC)、肺炎及早产儿视网膜病(ROP)等并发症发生率差异无统计学意义(P>0.05);两组患儿出现广泛性脑白质损伤(CWMD)、颅内出血(ICH)、局灶性CWMD及脑室周围白质软化(PVL)等脑受损概率差异无统计学意义(P>0.05);观察组患儿枕叶白质、内囊后肢、额叶白质及胼胝体压部FA数值均高于对照组,差异有统计学意义(P<0.05)。结论经过弥散张量成像FA检测显示早产儿采用rhEPO可保护其脑白质发育,可能和对少突角质细胞保护及加速其成熟有联系。
